Next Move Care
Purpose
Independent, private-pay advocacy for a defined healthcare decision or transition
Relationship
Chosen directly by the patient or an authorized representative; partners, family members, and chosen family may participate when the patient wants them involved and authority and privacy requirements are met
Strength
Question preparation, identity- and access-aware meeting support, cross-setting organization, and a written next-move roadmap
Limit
Does not diagnose, prescribe, provide hands-on care, practice law, control another organization, or guarantee an outcome

Hospital case management
Purpose
Support safe transition planning within a hospital encounter
Relationship
Part of the hospital team and its discharge workflow
Strength
Hospital-based coordination, discharge needs, referrals, and communication with the treating team
Limit
Scope and follow-up are generally tied to the hospital episode and available hospital resources

Health-plan navigation
Purpose
Help members understand benefits, networks, authorizations, and plan processes
Relationship
Provided by or through the insurer or health plan
Strength
Plan-specific coverage information and internal escalation routes

Works within the plan’s benefits, contracts, rules, and available programs

Geriatric care management
Purpose
Assess and coordinate longer-term needs, often for older adults and families

A clinical service delivered by appropriately licensed professionals, sometimes through a hospital, clinic, or home-based program

Specialized symptom support, goals-of-care discussion, and serious-illness communication
Limit
Availability, setting, eligibility, staffing, billing, and visit frequency vary; it is not an independent representative of the client

Hospice
Purpose
Provide interdisciplinary comfort-focused care for eligible people with terminal illness
Relationship
A hospice team operating under an elected hospice plan of care
Strength
Symptom support, nursing, psychosocial and spiritual care, equipment, medications related to the terminal diagnosis, and family support
Limit
Eligibility and services are tied to hospice requirements and the hospice plan of care

Nonmedical home care
Purpose
Provide personal care, companionship, supervision, meals, errands, or respite in the home
Relationship
A home-care organization or caregiver hired privately or through an eligible program
Strength
Practical day-to-day help with activities that support living at home
Limit
Usually does not provide independent clinical decision support or cross-system advocacy; services, training, and oversight vary

Private-duty nursing
Purpose
Provide extended one-to-one skilled nursing when ordered, arranged, and clinically appropriate
Relationship
A licensed nurse or agency working under applicable orders, scope, contracts, and payment arrangements
Strength
Longer-duration skilled observation and nursing tasks for people with substantial clinical needs
Limit
Cost, coverage, orders, staffing, service area, and licensing requirements apply; it is not replaced by advocacy

Skilled nursing & rehabilitation
Purpose
Provide facility-based nursing or rehabilitation after illness, injury, surgery, or functional decline
Relationship
A licensed facility working under admission criteria, orders, coverage rules, and an interdisciplinary plan
Strength
Daily nursing availability, rehabilitation, and structured transition support
Limit
Admission, coverage duration, therapy intensity, staffing, and discharge decisions depend on individual facts and responsible organizations

Concierge medicine
Purpose
Provide enhanced access to a physician or practice through a membership or retainer model
Relationship
A direct relationship with the participating clinician or medical practice
Strength
More access, continuity, or visit time depending on the specific membership
Limit
Clinician-centered and usually private pay; it does not automatically provide cross-setting advocacy, home care, or coverage

PACE
Purpose
Coordinate comprehensive medical and social services for eligible older adults who can safely live in the community
Relationship
An integrated Program of All-Inclusive Care for the Elderly serving a defined geographic area
Strength
Interdisciplinary care, medications, transportation, adult day services, and other covered supports through one program
Limit
Age, location, nursing-home-level eligibility, enrollment, and program availability apply

New services require their own legal and clinical foundation.
Next Move Care may later evaluate additional continuity, home-safety, wellness, medication-support, membership, or charitable programs. Nothing in that future direction is offered until the appropriate licensing, ownership, clinical accountability, insurance, privacy, contracts, staffing, and operating systems are confirmed.
The current service remains a nonclinical advocacy roadmapdelivered within a defined RN-led advocacy scope. Treating providers remain responsible for diagnosis, orders, treatment, and direct care.
