This website is designed to begin with basic contact information—not a medical record or detailed clinical story.
Information collected through the support form
The public form collects an email address, acknowledgment of the website boundaries, and optional name, phone, callback, voicemail, timing, and referral-source preferences. It does not provide a free-text field for a clinical story. Do not submit diagnoses, medical history, race or ethnicity, sexual orientation, gender identity, disability details, religion, immigration or refugee history, insurance information, Social Security numbers, financial information, or medical records through this form.
How the request is delivered
The callback form uses Formspree to deliver and store the limited request information for Next Move Care. Formspree may also send the request to the business email configured for the form. Ordinary internet transmission and email-based delivery are not appropriate for medical records. This website therefore does not accept document uploads.
Spruce communication channels
Next Move Care uses Spruce for its business phone and secure app-to-app messaging. Secure Spruce messaging is available at spruce.care/nextmovecarellc. Standard telephone calls, SMS, ordinary email, and fax require careful use because information may travel outside the secure app. Do not send medical records until Next Move Care confirms the approved channel, recipient, authorization, and scope.
Sensitive information and active clients
If a matter appears appropriate after the fit call, Next Move Care will first provide the written scope, privacy expectations, service agreement, and any necessary authorization. If records are genuinely needed, the client will receive instructions for an approved secure method. Do not send records by ordinary email, text message, or social-media message.
AI and chatbot boundary
Do not enter patient records, passwords, authentication codes, or a detailed clinical story into ChatGPT or another AI chatbot for delivery to Next Move Care. The public website and any later approved client-record system are separate from conversational AI tools.
Communication with other people or organizations
Hiring Next Move Care does not automatically authorize communication with a hospital, clinician, payer, facility, family member, or other third party. Communication on a client's behalf requires appropriate written permission and remains subject to the receiving organization's verification requirements.
Referral-source measurement, tracking, and cookies
The optional “How did you find us?” field helps Next Move Care understand which educational and community channels are useful. The site does not intentionally use advertising trackers or analytics cookies. The website host and security provider process ordinary technical request data—such as IP address, browser details, requested page, and time—and may set a strictly necessary security cookie to detect automated or abusive traffic. Formspree may process similar technical data when the support form is submitted. If analytics or advertising tools are added later, this notice should be updated before collection begins, and sensitive form content must never be placed in general website analytics.
Your choices
You may omit a phone number and use email for the initial response. You may decline voicemail permission. Active clients may request correction of inaccurate contact information and may end services according to the written service agreement.
Choose a private way to respond
Use an email address or phone number that you can access privately. If voicemail could create risk, leave voicemail permission unchecked. Do not include medical or identity details in the public form.
HIPAA and Business Associate Agreements
Whether a particular engagement is governed by HIPAA—and whether Next Move Care is acting as a business associate—depends on the facts, parties, and services involved. This notice does not claim blanket HIPAA compliance. The Spruce organization includes a Business Associate Agreement with Spruce when applicable, but that vendor agreement does not by itself determine Next Move Care's legal role in every engagement. Covered entities and professional partners may request BAA or privacy review before any PHI is exchanged.
Identity, culture, disability, and chosen-family privacy
No one is required to disclose race or ethnicity, sexual orientation, gender identity, disability, religion, immigration or refugee history, or family structure to request service. During accepted work, Next Move Care will ask what name, pronouns, relationships, access needs, and identity information the client wants used or shared within the authorized scope. Applicable law, immediate safety obligations, and the receiving organization's verification requirements may still apply.
