- A discharge or care-planning deadline is approaching and the family needs independent preparation.
- A patient or legal representative wants help organizing questions before a care conference.
- The proposed plan depends on family help, equipment, transportation, placement, or services that remain unclear.
- A patient or chosen family feels excluded or overwhelmed because race, gender identity, disability, language, age, culture, migration history, or family structure is not being respected.
- The family needs education and advocacy—not diagnosis, treatment, legal representation, or emergency response.
Use the person's requested name and pronouns. Share identity, disability, language, family, or immigration information only with permission and only when relevant to the introduction.

The person may request a free first conversation without sending records or a clinical history.

Do not send protected, confidential, or case-specific information unless an appropriate authorization and secure method are confirmed.

Next Move Care does not pay or accept compensation for referrals and does not promise placement, coverage, eligibility, legal, or clinical outcomes. A referral does not create a client relationship or guarantee acceptance. Potential matters are screened for fit, authority, conflicts, capacity, and scope.
