How intake works

A submitted request does not accept, schedule, or insure a service and does not create a client or clinician-patient relationship.

    Share only contact information, the city and state, requested setting, broad service interest, and general timing. Do not send private health details.

    Confirm fit and availability

    Next Move Care reviews age eligibility, scope, professional authority, local requirements, insurance, capacity, and whether the requested setting can be accepted.

    Complete secure intake

    If the request can move forward, Next Move Care opens a secure Spruce conversation and provides only the intake modules and documents needed for the accepted scope.

After preliminary acceptance

The detailed intake moves to Spruce.

Next Move Care may securely request a client profile, residence-visit information, a medication or supplement list, caregiver and resource details, administrative appeal documents, and an emergency contact plan. Only the modules needed for the written scope should be used.

Completed intake documents and private health information should be exchanged through secure app-to-app Spruce messaging, not ordinary email, SMS, or this website form.

  • No public record or photo uploads
  • No street address, date of birth, insurance ID, or medication list on the public form
  • No service begins before written acceptance and a signed agreement
  • Separate authorizations are used when communication with another person or organization is needed
